We welcome your interest in Desert Christian Middle School.  The application procedure for 2008 – 2009 student admission follows.  Please call my office if you should have any questions.  I look forward to meeting you.

   I.    APPLICATION FOR ADMISSION

A.      Please read the Statement of Faith and Philosophy of Education.

B.       Complete the Family Application form (one per applicant) and attach a recent photo.

B.       Complete the Home School Family Information form.

C.      Complete the Special Health/Learning Information form.

D.   Include the applicant’s most recent:

v     Grades (optional)

v     Standardized Test Scores                                                                                               

v     Immunization Records                                                                                                                   Dennis O'Reilly, Principal

E.   Submit completed forms and records with the non-refundable application fee of $50.00 to:

Attn: Admissions Coordinator

F.   Complete the top portion of the forms indicated below and give them (along with a school-addressed, stamped envelope) to the appropriate person as indicated on each form. These forms must be completed and mailed directly to the middle school office.

II.        CANDIDACY FOR ACCEPTANCE

A.      The application is considered complete when all items listed above are completed and received at Desert Christian Middle School.

B.       After Admissions Committee review, qualified candidates will be contacted by mail or phone to schedule a family interview.

C.      A proficiency exam may be required for your child in the areas of Language Arts and Math.

D.      Parents will be notified by mail of the final acceptance decision by the end of March 2008.

 

III.      ENROLLMENT AS A STUDENT

A.      The DCS Tuition Contract (included with acceptance letter) and $350.00 non-refundable registration fee must be received in the middle school office within two weeks of receipt of the items to guarantee your child’s enrollment in the middle school. 

B.       Families will be notified of the dates for the New Student Welcome and Orientation in July 2008 as well as the Parent Orientation in August 2008.  Attendance is encouraged at these important events.
